Membership Terms and Conditions

1. REQUIREMENTS: Member must be 18 years of age and provide a valid driver’s license, social security
number, and either (a) a major credit card, or (b) a current secondary I.D. type.

2. OTHER USERS: Upon application, member must indicate the names of other users, if any, who are authorized
by member to rent items on this Starlite membership. Member is fully responsible for the timely and safe return of all
items rented through the use of his or her membership card.

3. RENTAL FEES: All items will be rented at a published fee per day per rented item. Rental fees begin and are
paid on the date the rented items are picked up.

4. LATE FEES: Additional rental fees will be applied to any rental item not returned by the published due date and
time.

5. FAILURE TO RETURN: If member does not return any rented item within 15 days from the due date, in
addition to any late fees, member will be responsible for the full replacement cost of the rented item(s).

6. DAMAGE: All rental items must be returned in the same condition as when they were rented, except for normal
wear and tear. If any rented item is returned damaged, member will be responsible for the cost of repair, not to
exceed the full replacement cost of the item.

7. LOST, STOLEN, DESTROYED: If member is unable to return any rented item because it has been lost,
stolen or destroyed, member will be responsible for the full replacement cost of the item.

8. LOSS OF MEMBERSHIP CARD: Member is responsible for all items rented through the use of his/her
membership card, whether or not such use is authorized by member. If a membership card is lost or stolen, member
must immediately notify Starlite in writing. The member will not be responsible for any use of the card after Starlite
has received and signed a written notice. A new membership card and number will be issued to member.

9. MEMBER’S PROPERTY: Starlite will not be responsible for any damages to member’s property that might
be caused by any rented items.

10. AMENDMENTS: Starlite reserves the right at any time and without notice, to amend the Terms and
Conditions of Membership.

Starlite reserves the right to refuse memberships.
